IOWA CRIMINAL HISTORY RECORDS
Effective July 1, 1996, Iowa criminal history information became available to members of the public. The Division of Criminal Investigation is the central repository for criminal history information for the State of Iowa.
The fee for each request is $13.00 per surname for every mail-in/mail-back request and $15.00 for every fax-in/fax-back request. A fax back request requires a credit/debit card as the method of payment. This is for an Iowa check only, and is based on name and date of birth. Without fingerprints, a positive identification cannot be assured. No other state or federal agency records are included in this record check.
Only the Division of Criminal Investigation can perform a statewide criminal history records check and release results to non-criminal justice agencies. For further information, contact:

To request the forms necessary to do a record check, call (515) 725-6066, or
download the forms here. Each form is available in either Word for Windows 6.0 format (with .doc as the extender on the file name) or in Portable Document Format (with .pdf as the extender). A billing form must be submitted, along with one record check request form for each surname on which a check is being requested. Note also that prospective employers, licensing authorities, and organizations considering a person for volunteer work may need to obtain a waiver from the person about whom the record check is being completed. A
signature line for granting the waiver is located at the bottom of the first
page of the record check request form, and an explanation of the waiver is contained
on the second page (reverse on the printed version of the form).
