Law Enforcement Agencies Investigate Death of Boone Woman
Boone, IOWA – On Sunday, January 29, 2012 at approximately 8:02 PM, the Boone County Communications Center received a 911 call in reference to an unresponsive female at 1210 Southview Court in Boone, Iowa. The Boone Police Department responded to the residence and found Emily Fazzino, 34, unresponsive. Fazzino was transported to the Boone County Hospital where she was pronounced dead.

Hastings, IOWA --- Investigators with the State Fire Marshal Division and the Mills County Sheriff’s Office are looking for 30 year old Jason Anthony Vasquez. Vasquez is wanted on an arrest warrant for 2nd Degree Arson and 2nd Degree Burglary, after an incident that occurred on February 12, 2011 at approximately 7:06 AM.

Man Charged with Identity Theft after Falsely Implicating Secretary of State in Illegal Activity
Des Moines, IOWA --- Today, Friday, January 20, 2012, Zachary Edwards, age 29, from Des Moines, Iowa, was arrested and criminally charged with Identity Theft, an Aggravated Misdemeanor (Iowa Code 715A.8(2)). Edwards turned himself in to Iowa Division of Criminal Investigation (DCI) agents this afternoon at the Polk County Jail. He was then booked into the jail with a set bail of $2,000, cash or surety.

Vehicle Found, Alleged Suspect Believed Dead
Mt. Ayr, IOWA – This evening, 40 year old Robert Taylor's vehicle was located at the eight mile marker on the side of the road along I-29, northbound. A subject matching the description of Taylor is deceased due to a self-inflicted gunshot wound. However, positive identification won't be made until the autopsy is completed by the State Medical Examiner's Office.

Mt. Ayr, IOWA – This morning at approximately 2:06am, the Ringgold County Communications Center received a 911 call from 2339 – 280th Street in Mt. Ayr. A juvenile inside the home called and reported a domestic altercation between Robert Taylor, 40, and Lori Yeager, 45. The responding deputies found Lori Yeager dead inside the home; the victim of an apparent homicide. Robert Taylor was not located at the home.

Law Enforcement Conduct Death Investigation in Huxley
Huxley, IOWA --- On Saturday, January 7, 2012 the Huxley Police Department received a phone call in reference to a welfare check on a 22 year old female. Huxley Police responded to 514 W. Main Ave, Apt. B, in Huxley and learned that the female in question had given birth to twins at her residence. The twins did not survive. The female was taken to a local hospital.

STATE FIRE MARSHAL REPORTS 186 LIVES SAVED BY SMOKE ALARMS IN 2011
Des Moines, IOWA --- The Iowa Department of Public Safety, State Fire Marshal Division reports 2011 ended with 186 people saved from fire death or injury by working smoke alarms. Most of the fires from which the citizens escaped, occurred in the evening when the occupants were sleeping. The simple sound of a fire alarm saved their lives.

DCI Investigates Johnson County Death
Johnson County, IOWA --- On October 1, 2011, the Johnson County Sheriff's Office made a request to the Iowa County Sheriff's Office to arrest 26 year old Crystal Hawkins at a North English residence on a warrant/mittimus to serve 30 days in the Johnson County Jail. Upon her arrival and prior to being booked into jail, Ms. Hawkins became ill and was transported to University of Iowa Hospitals and Clinics and admitted as a patient. On October 2, 2011, the Johnson County Medical Examine advised Ms. Hawkins died while being treated at the University of Iowa Hospitals. The Johnson County and Iowa County Sheriff's Offices requested the Division of Criminal Investigation conduct an independent investigation into the death of Ms. Hawkins.

Law Enforcement Officials Conduct Death Investigation in Coon Rapids, Iowa
Des Moines, IOWA --- On Saturday, September 3, 2011, approximately 1:00 p.m. police and paramedics were called to a residence in Coon Rapids for a male that was involved in an altercation. The male was identified as 28 year-old William Smith. Smith was taken to St. Anthony Regional Medical Center in Carroll for treatment and later flown to Iowa Methodist Medical Center in Des Moines. Smith was pronounced dead late Sunday afternoon, September 4, 2011. An autopsy is scheduled for Tuesday in Des Moines.

Several people have been admitted to the University of Iowa Burn Unit with serious burns as a result of using decorative, gel fire pots. Nationally, there have been over 37 reported accidents, with 23 of them injuring consumers.

Warren County, IOWA --- Autopsy tests have confirmed the remains of a deceased male found on July 31, 2011 in the burned home of Bill and Kay Wood, are that of 79 year old James "Bill" Wood. Autopsy results also indicate Bill Wood died of multiple gun shot wounds. The manner of death is considered homicide.

DCI Releases Finding in Black Hawk County Inmate Death
Black Hawk County, Iowa --- On December 1, 2010, at approximately 2:00 PM, 18 year old Justin Johnson, an inmate of the Black Hawk County Jail, notified a jail deputy that he was feeling ill. The deputy entered the cell and found that Johnson was having difficulty breathing. Jail staff requested an ambulance for Johnson and he was taken to Allen Hospital in Waterloo, where doctors declared him deceased.

Driver Found In Eastern Iowa Hit and Run
Tiffin, Iowa --- The Iowa State Patrol has located the driver involved in a February 13, 2011, hit and run on I-80 near Tiffin, Iowa. Eagle Grove, IOWA --- On Friday, January 7, 2011, emergency personnel were called to 208 North Wright Avenue in Eagle Grove with a report of an unresponsive child. Mikayla Valentine, age 5, was taken by air ambulance to University of Iowa Hospitals and Clinics. She was pronounced dead at approximately 9:00 a.m. on Saturday, January 8, 2011. Today, Monday, January 10, 2011, an autopsy is scheduled to be performed at the Johnson County Medical Examiner’s Office.
