Application Form for Licensing
The application form to be used to apply for a
license as an electrician or an electrical contractor is now available.
There are four documents here in pdf format. Please download all four:
- Pro-Rated Schedule of Electrician License Fees
- Cover
letter addressed to all those working as electricians in Iowa.
- Detailed
instructions for completing the application. Please read these before
starting to fill in the application.
- Application
form. Please complete the application form as thoroughly as you
are able after reading the instructions. Also note that the affidavit
required to be filed with an application for a Class B license is included
in the application form.
NOTE: Payment of the license fee by check, money order, or warrant
must accompany the submitted application form.
Completed applications, accompanied by payment of the license fee,
must be submitted to:

AN ELECTRICIAN NOT LICENSED BY THE LOCAL JURISDICTION IN WHICH
THE ELECTRICIAN IS WORKING MUST HAVE APPLIED AND PAID THE FEE FOR
A STATE LICENSE IN ORDER TO PERFORM ELECTRICAL WORK ON OR AFTER
JANUARY 1, 2008.
